CD28分子存在于大多数T细胞表面，被认为是一种T细胞特有的表面分子。T细胞对抗原的反应性主要通过CD3/TCR复合物介导，但也有赖于T细胞表面其他分子的协同。ＣＤ２８与其天然配基Ｂ７结合可发挥共刺激作用，作为第二信号介绍Ｔ细胞激活，促进Ｔ细胞增殖及分泌多种细胞因子并介绍导Ｔ，Ｂ细胞粘附。在体外T淋巴细胞激活扩增中抗人CD28单克隆抗体通过第二信号通路发挥共刺激作用。
